Jeans Day Business Participation Form

Use the form below to apply for participation as a business in Jeans Day. Be sure to designate a coordinator within your business to collect the money and hand out stickers. You'll receive a letter stating the chosen charity of the month and enough Jeans Day stickers for all participating employees. You'll also receive Jeans Day posters to hang in your building and your business name will appear in the full page Jeans Day ad publishing in the Bismarck Tribune each month. It is important that you fill out all the fields to ensure your request is quickly and properly processed.
